Job Summary

We are seeking an experienced Advanced Practice Provider to join our team at Glenbrook Hospital in Glenview, IL. As an Advanced Practice Provider, you will be responsible for providing comprehensive health assessments, differential diagnosis, and prescribing treatments for patients with advanced heart failure.

Key Responsibilities
  • Provide high-quality patient care and education to patients and families
  • Work collaboratively with interdisciplinary teams to develop and implement patient care plans
  • Conduct thorough health assessments and develop treatment plans to improve patient outcomes
  • Prescribe medications and treatments as needed
Requirements
  • Current Physician Assistant or Advanced Practice Nurse License in the State of Illinois
  • DEA & Illinois Controlled Substance License
  • Bachelor's degree required; Master's degree preferred
  • BLS for Healthcare Provider certification
  • Prior experience in cardiology preferred
