Primary Care Physician or Nurse Practitioner/Physician Assistant

LifeLong Medical Care, a leading Federally Qualified Health Center in Northern California, is seeking a dedicated and compassionate Primary Care Physician or Nurse Practitioner/Physician Assistant to join our multidisciplinary team at the Over-Sixty Health Center. Our ideal candidate is committed to providing high-quality outpatient primary care to underserved populations.
We are looking for a skilled healthcare professional who is passionate about building a collaborative team model of care. The successful candidate will work alongside our primary care providers, RNs, social workers, psychiatrists, podiatrists, and other healthcare professionals to provide comprehensive, patient-centered care. Our team utilizes a multidisciplinary approach to ensure that every patient receives the best possible care.
The Primary Care Physician or Nurse Practitioner/Physician Assistant will be responsible for providing preventive, urgent care, and primary care health services to our patients. This includes performing clinical assessments, ordering diagnostic evaluations, and planning programs of care. The selected candidate will also provide verbal and written education and counseling to patients, families, and caregivers.
We offer a competitive salary range of $197,000-$218,000 per year for MD/DO, depending on years since finishing residency, and $123,000-$144,000 per year for NP/PA, depending on years since licensed, as well as an excellent benefits package, including medical, dental, vision, Flexible Spending Accounts, dependent and domestic partner coverage, 403(b) retirement savings plan, and loan repayment programs. LifeLong qualifies for federal and state loan repayment programs (HPSA score is 21).
