The Patient Care Coordinator (LPN) plays a vital role in assisting the Patient Care Manager by coordinating a dedicated team to ensure the delivery of exceptional care to home health patients within the designated area. This position involves:
- Ensuring that all team members are well-trained and informed
- Maintaining a commitment to providing quality care efficiently, in alignment with physician directives and State/Medicare regulations
Experience Requirements
- A minimum of one year of experience in home health care is required.
- Possession of a current LPN/LVN license in the state of practice, free from restrictions.
- Current CPR certification.
- A valid driver's license, vehicle insurance, and reliable transportation or access to public transit.
