Key Responsibilities

The Patient Care Coordinator (LPN) plays a vital role in assisting the Patient Care Manager by coordinating a dedicated team to ensure the delivery of exceptional care to home health patients within the designated area. This position involves:
  • Ensuring that all team members are well-trained and informed
  • Maintaining a commitment to providing quality care efficiently, in alignment with physician directives and State/Medicare regulations

Qualifications

Experience Requirements

  • A minimum of one year of experience in home health care is required.

Licensure Requirements

  • Possession of a current LPN/LVN license in the state of practice, free from restrictions.
  • Current CPR certification.
  • A valid driver's license, vehicle insurance, and reliable transportation or access to public transit.
