Key Responsibilities:- Admitting Patients: You will be responsible for admitting patients to our hospice service, ensuring that they receive the necessary care and support.
- Evaluating Patients: You will evaluate patients as needed, assessing their physical, emotional, and psychosocial needs.
- Developing Care Plans: You will develop comprehensive care plans that address the unique needs of each patient and their family.
- Documentation: You will complete all necessary paperwork and documentation, ensuring that patient records are accurate and up-to-date.
- Education: You must have a degree from an accredited school of nursing, with a BSN preferred.
- Licensure: You must be licensed to practice as a Registered Nurse in the state of Connecticut.
- Experience: You must have a minimum of two years of nursing experience, with experience in home health care and hospice/end-of-life care preferred.
- Certifications: You must be certified as a Hospice and Palliative Care Nurse (CHPN) or be willing to obtain certification.
