About the Role

FeldCare Connects is seeking a skilled Physical Therapist to deliver high-quality home healthcare to patients of all ages. As an independent contractor, you will have the flexibility to control your own schedule and work in your preferred coverage areas.

Key Responsibilities
  • Provide one-on-one treatment to patients in their homes, observing, recording, and reporting their response to treatment and changes to their condition.
  • Develop and implement individualized plans of care, determining the best course of treatment for each patient.
  • Work collaboratively with our team to ensure seamless care coordination, scheduling, and administrative support.
  • Utilize our intuitive Electronic Medical Record system to document patient information and treatment plans.
Requirements
  • Current Physical Therapist license issued by the state Board of Physical Therapy.
  • Completion of an accredited Physical Therapy program.
  • Reliable personal transportation and a smartphone.
  • Bilingual skills a plus.
