Position Overview

The Registered Nurse Care Coordinator reports directly to a Clinical Supervisor and is tasked with the coordination of care among clinical staff, patients, families/caregivers, and healthcare providers. Responsibilities include managing admissions, recertification assessments, transfers/resumptions of care, and discharge assessments, along with the necessary documentation.

Key Responsibilities

  • Oversees the direct supervision of CNAs, LPNs, and RNs in the field, including training and re-education on clinical skills and procedures as needed.
  • Creates individualized care plans tailored to each client’s needs.
  • Offers support, guidance, and education to patients and their families/caregivers to facilitate progress toward health goals.
  • Ensures timely and accurate completion of all clinical documentation, adhering to agency policies and state regulations.
  • Participates in ongoing quality improvement initiatives by reporting incidents, injuries, and infections.
Qualifications

  • Current state licensure as a Registered Nurse; BSN preferred.
  • At least one year of nursing experience required (two years preferred).
  • Exhibits strong leadership abilities with effective supervisory and interpersonal skills.
  • Detail-oriented, adaptable, and possesses excellent organizational and time management skills.
  • Current CPR certification and applicable health certificate.
  • Valid state driver's license and automobile insurance.
Additional Information

The salary range for this role is $70,000-$75,000 annually, with eligibility for a monthly auto and cell phone allowance totaling $4,200.00 annually.
