Job Summary:

A Physical and/or Occupational Therapist, and Speech Therapist administers occupational therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physicians' orders and plan of care under the direction and supervision of the Clinical Services Manager.

Responsibilities:
  • Improves or minimizes residual physical disabilities of the patient.
  • Periodically participates with all other home care personnel in patients' care planning.
  • Provides prescribed physical or occupational therapy.
  • Responsible for initial assessment, plan of care, maintenance program development and modifications, and reassessments every 30 days.
  • Consorts with physicians regarding change in treatment.
  • Prepares clinical and progress notes.
  • Instructs patient/family members in home programs and fine motor movement exercises.
  • Helps develop the plan of care and revises as necessary.
  • Performs all skilled procedures.
  • Writes report to physician regarding the patient's progress.
Qualifications:
  • Graduate of a Physical and/or Occupational Therapy curriculum accredited by the American Medical Association.
  • Currently Licensed and Registered in the state practicing.
  • Two years preferred. Homecare a plus.

Work Environment:

Works indoors in Agency office and patient homes and travels to/from patient homes.

Requirements:
  • Experience in outpatient and inpatient settings.
  • Proficiency in critical care and acute care environments.
  • Strong patient care skills with a focus on individualized treatment.
  • Knowledge of medical terminology, anatomy, and physiology.
  • Ability to provide compassionate care and support to patients undergoing rehabilitation.
