As a Clinical Pharmacist PRN at SSM Health, you will play a vital role in providing pharmaceutical care to patients, verifying medication orders, and dispensing pharmaceuticals in accordance with federal, state, and local regulations and hospital policy.
Responsibilities- Ensures safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, lab value monitoring, and dose/therapy adjustment as applicable.
- Documents medication-related plans and interventions in electronic medical records.
- Provides clinical consultation delivering best-practice, evidence-based, and complete information to practitioners, making recommendations and encouraging safe and cost-effective pharmacotherapy for patients through multi-disciplinary collaboration.
- Oversees the medication dispensing process to ensure accurate and timely delivery of optimal pharmacotherapy to patients, including supervision of pharmacy technicians and/or interns who support the role of the pharmacist in the drug delivery system.
- Precepts pharmacy students and residents, as applicable.
- Works in a constant state of alertness and safe manner.
- Performs other duties as assigned.
- Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from an accredited school of pharmacy.
- No experience required.
- Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
- Frequent sitting, standing, walking, reaching, and repetitive foot/leg and hand/arm movements.
- Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
- Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.
- Frequent keyboard use/data entry.
- Occasional bending, stooping, kneeling, squatting, twisting, and gripping.
- Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
- Rare climbing.
- Pharmacist, Registered - Illinois Department of Financial and Professional Regulation (IDFPR).
- Pharmacist - Missouri Division of Professional Registration.
- Pharmacist - Oklahoma State Board of Pharmacy.
- Pharmacist - Wisconsin Department of Safety and Professional Services.
