About the Role

We are seeking a highly skilled Speech Language Pathologist to join our team at CenterWell Primary Care. As a key member of our home health care team, you will play a vital role in providing personalized care to patients in their homes.

Key Responsibilities
  • Evaluate and provide speech/language pathology services to patients in their homes or facilities
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Conduct assessments and provide treatment to patients with speech, language, hearing, and swallowing disorders
  • Administer and interpret diagnostic tests and provide therapeutic treatments
  • Document patient progress and communicate with healthcare teams
  • Provide education and training to patients and their families on alternative communication systems and strategies
  • Participate in care coordination activities and discharge planning
Requirements
  • Meet the education and experience requirements for Certification of Clinical Competence in Speech-Language Pathology or Audiology
  • Minimum of six months experience as a speech therapist/speech-language pathologist
  • Home health experience preferred
  • Current and unrestricted license
  • Current CPR certification
  • Good organizational and communication skills
  • Valid driver's license and reliable transportation
