About the Role

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Mental Health Partners. As a key member of our integrated health home, you will play a vital role in providing essential support and guidance to our diverse clientele facing complex mental health, substance use, and environmental challenges.

Key Responsibilities
  • Manage a caseload of clients and their diverse needs, providing individualized support and guidance to promote health and wellness.
  • Collaborate with a multidisciplinary team to develop and implement comprehensive treatment plans, ensuring seamless coordination of care.
  • Provide education and support to clients and their families, promoting self-advocacy and empowerment.
  • Develop and maintain relationships with community resources and providers, ensuring access to necessary services and support.
  • Participate in ongoing training and professional development to enhance skills and knowledge in areas such as motivational interviewing, dialectical behavior therapy, and cognitive behavioral therapies.
Requirements
  • Bachelor's degree in Psychology, Social Work, Counseling, or a related field.
  • One year of experience in the behavioral health field, with a focus on working with adults in an outpatient setting.
  • Strong knowledge of clinical modalities, including motivational interviewing, dialectical behavior therapy, and cognitive behavioral therapies.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work independently and as part of a team, with a strong commitment to collaboration and client-centered care.
