Job Summary

We are seeking a skilled Nurse Planner to join our team. In this role, you will be responsible for coordinating patient services through an interdisciplinary process, ensuring a clinical and psychosocial approach to care throughout the continuum.

Key Responsibilities
  • Conduct concurrent case management to assess patient appropriateness for admission, continued hospitalization, and level of care.
  • Develop and implement discharge plans, beginning at admission or prior to admission, and reassessing ongoing throughout hospitalization.
  • Monitor and report Quality and Risk Management issues as necessary.
  • Collaborate with staff, physicians, patients, and families in the Emergency Department to achieve desired quality outcomes and identify/controls inappropriate resource utilization.
  • Establish patients with resources to ensure a safe transition of care from the Emergency Department, including arrangement of follow-up care and referrals to community programs.
  • Facilitate patient and family education, promoting continuity of care to achieve optimal patient outcomes.
Requirements
  • Valid, unrestricted RN License in the state of Colorado or Master of Social Work (MSW) Degree.
  • Ability to commit to working 1 weekend day per pay period (1 weekend day every 2 weeks).
  • Experience in a fast-paced environment such as Urgent Care, Emergency Department, or Medical/Surgical care settings.
