Job SummaryWe are seeking a skilled Nurse Practitioner/Physician Assistant to join our team at La Clinica de La Raza. As a primary care provider, you will deliver patient-centered care to a resilient patient population, working collaboratively with a cross-functional team of providers to achieve optimal health outcomes.
Responsibilities- Provide medical triage, care management, and patient education to a panel of patients
- Make referrals to specialty providers and follow-up to ensure patients' bio-psychosocial needs are addressed
- Deliver evidence-based care that supports improved health outcomes, exceptional patient experience, and efficient use of resources
- Prescribe medications, treatments, or therapies and order medical diagnostic or clinical tests
- Collaborate with interdisciplinary team members to improve clinic flow, patient scheduling, and service delivery
- Current license from the California Board of Registered Nursing or California Physician Assistant Committee
- Board certification from the American Academy of Nurse Practitioners or other national certifying bodies
- Minimum of 2 years clinical experience
- Knowledge of Electronic Health Record programs, such as NextGen or Epic
- High degree of interpersonal competence with staff and patients
