This position is integral to the mission of St. Mary's Healthcare System for Children, ensuring the delivery of exceptional care by adhering to regulatory standards, fulfilling agency requirements, and maintaining necessary certifications.

Key Responsibilities:

  1. Deliver expert physical therapy services in alignment with Individualized Education Programs (IEP) and Individualized Family Service Plans (IFSP) as prescribed by physicians.
  2. Collaborate with families, educators, and other professionals to enhance the effectiveness of treatment plans.
  3. Implement therapeutic interventions aimed at developing and restoring functional abilities while maximizing patient performance through tailored physical therapy techniques.
  4. Monitor, document, and communicate patient responses to treatment and any changes in their condition to the relevant healthcare team members.
  5. Provide guidance and education to patients, families, and healthcare personnel, including physical therapy assistants and rehabilitation aides, on the physical therapy process.
  6. Conduct thorough evaluations and ongoing assessments of patient rehabilitation needs, adjusting care plans as necessary.
  7. Ensure that care plans comprehensively address all identified issues or provide justifications for any omissions.
  8. Develop measurable patient outcomes and discharge plans for each individual.
  9. Timely submit progress notes, evaluations, and necessary documentation in accordance with service mandates and insurance requirements.
  10. Modify treatment plans based on the evolving needs of the patient, securing necessary approvals for changes.
  11. Regularly assess the effectiveness of services provided, ensuring evaluations occur at least four times annually.
  12. Educate and support patients and their families in activities that facilitate the achievement of treatment goals.
  13. Recommend appropriate materials and equipment to assist in meeting treatment objectives.
  14. Engage in interdisciplinary meetings related to patient care, including IEP and IFSP discussions.
  15. Initiate necessary diagnostic, preventative, and rehabilitative measures to ensure patient safety and care.
  16. Coordinate with other healthcare professionals to maintain continuity of care for patients.
  17. Document patient care progress accurately and promptly, adhering to established policies.
  18. Participate in professional development and in-service training programs.
