Responsibilities

As a Registered Nurse with our team, you will work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes. Your responsibilities will include:

  • Conducting patient home visits
  • Assessing, planning, directing, and evaluating each patient's care plan to ensure it meets their hospice needs
  • Managing emergency situations in a prompt, precise, and professional manner
  • Counseling patients and their families on end-of-life needs
  • Completing medication administration and treatment needs for all assigned hospice patients
  • Providing nursing care in accordance with applicable regulations and Company policy
  • Preparing and maintaining accurate and complete clinical and progress notes on a timely basis
Qualifications
  • Active RN state license
  • Valid drivers' license
  • Two or more years of applicable nursing experience
  • Nursing supervisory/leadership experience (preferred)
