POSITION SUMMARY:
The Physical Therapist is responsible for delivering therapeutic interventions to residents referred by their healthcare providers. This role involves assessing patient needs and developing tailored treatment plans to facilitate recovery and improve functional abilities.
KEY RESPONSIBILITIES:
1. Evaluate patient referrals and physician directives to determine appropriate therapeutic interventions.
2. Develop and implement individualized treatment plans based on patient assessments, goals, and rehabilitation potential.
3. Maintain accurate and thorough documentation of patient evaluations, treatment progress, and discharge summaries.
4. Collaborate with the interdisciplinary team to ensure cohesive patient care and communicate effectively with family members regarding treatment progress.
5. Supervise and guide physical therapist assistants in executing treatment protocols.
6. Uphold a productivity standard of 85% while ensuring high-quality patient care.
7. Participate in departmental meetings and contribute to continuous improvement initiatives.
WORK ENVIRONMENT:
This position operates within a clinical rehabilitation setting, requiring physical stamina to engage with patients and staff throughout the day. Responsibilities include lifting and moving patients as necessary, necessitating physical strength and coordination.
QUALIFICATIONS:
A degree in physical therapy from an accredited institution and a valid New York State license to practice are essential. The ideal candidate will possess strong analytical skills to assess patient conditions and communicate effectively with both patients and healthcare teams.
