We are seeking a skilled Nurse Practitioner to join our team as a Float Nurse Practitioner for our PACE program. As a key member of our multi-disciplinary team, you will provide primary health care to our program participants, focusing on disease prevention, wellness, and management of chronic and episodic illness.
Responsibilities- Provide initial and periodic assessments of participants and identify and document problems/issues in medical care.
- Evaluate episodic illnesses and implement appropriate diagnostic and therapeutic interventions.
- Collaborate with physicians and other team members to manage episodic and chronic medical needs of participants.
- Perform diagnostic, therapeutic, and nursing interventions as needed.
- Implement plans of care in a safe and effective manner.
- Participate in multi-disciplinary team meetings and contribute to the development of individualized plans of care.
- Evaluate the effectiveness of previously implemented plans of care and make recommendations for improvement.
- Conduct home and nursing home visits as needed to provide care and support to participants.
- Facilitate the continuum of care for sub-acute and long-term nursing home residents.
- Maintain accurate and precise documentation of all services provided.
- Identify utilization and service delivery problems and participate in their resolution.
- Participate in quality assessment activities, including record reviews, outcome measurement, and review/revision of policies and procedures.
- Provide education and training to participants, caregivers, and staff on relevant topics.
- Act as a resource to program staff and participate in defining nursing standards and policies.
- Serve on appropriate committees and participate in staff development activities.
- Master's degree in Nursing required, with a focus on Gerontology or a related field.
- Licensed by the Commonwealth of Massachusetts Board of Registration in Nursing.
- Valid driver's license and clean driving record required.
- National Board Certification as a Geriatric, Adult, or Family Nurse Practitioner.
- One to three years of Nurse Practitioner experience preferred, with at least one year of experience in geriatric care.
