Abode Care Partners is a leading provider of integrated medical services, delivering quality care to individuals in various settings. We are seeking an experienced Nurse Practitioner to join our team as a Home Healthcare Medical Specialist.
About the Role
As a Home Healthcare Medical Specialist, you will be responsible for providing personalized medical care to seniors and patients with complex needs in their home settings. Your role will involve planning, directing, and evaluating patient medical needs, collaborating with healthcare professionals, and ensuring accurate medical records.
Key Responsibilities
- Perform evidence-based medical care by examining and treating chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses.
- Order and interpret laboratory and diagnostic testing as needed, ensuring accurate diagnoses and effective treatment plans.
- Collaborate with caregivers and other healthcare professionals to provide interdisciplinary care and maintain comprehensive plans of care.
- Provide referrals to other healthcare practitioners when necessary, prioritizing patient needs and outcomes.
- Ensure patient medical records are up-to-date, maintaining confidentiality and adhering to HIPAA regulations.
Qualifications
- Active state Nurse Practitioners license
- Current Nurse Practitioner National Board Certification
- Active state DEA licensure
- Master's degree in Nursing (MSN)
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
- Strong interpersonal communication skills – verbal and written
- Excellent time management skills with accountability for practitioner's daily workflow
- Proficiency in medical instruments and equipment
- Strong knowledge base of HIPAA and OSHA regulations
