  • Collaborate with patients, families, and healthcare teams to ensure seamless transitions of care
  • Document patient information accurately and timely

Requirements:

  • Graduate of an accredited school of nursing
  • Current RN licensure in California
  • Basic Life Support (BLS) certification
  • Valid driver's license and auto insurance
  • 2 years of recent experience in home health care

Working Conditions:

This is a full-time position, working 40 hours per week, Monday through Friday. Rotating weekends may be required.
