Key Responsibilities:
  • Participate in and/or lead iterative goals of care conversations with patients in various stages of serious illness, using advanced clinical communication and a whole-person approach.
  • Create informative and clear documentation, particularly for patients with high complexity in need of contingency planning.
  • Proactively partner and collaborate with TEC colleagues across multiple disciplines and settings to create integrative, robust care plans.
  • Work closely with provider colleagues to ensure that palliative care case management is coordinated, particularly when patients are dealing with multiple co-morbidities, complex and/or serious illnesses, particularly towards end of life.
  • Conduct visits with patients and families in the clinic, telephonically, at in-home visits, at hospitals, in skilled nursing facilities, in adult family homes and other locations as appropriate.
Education and Facilitation:
  • Grows provider and staff competency in advanced practice communication skills, serious illness management, and palliative care approach and philosophy of care.
  • Act as a palliative resource for teams across TEC.
  • Provide informal and formal education for providers and staff around serious illness management, end of life care, clinical communication, effective goals of care conversations, interdisciplinary collaboration, and other related topics.
Requirements:
  • Graduation from an accredited school of nursing.
  • Basic Life Support for Healthcare providers (AHA) or CPR/AED for the Professional Rescuer (American Red Cross).
  • Washington State Registered Nurse license.
  • 3+ years of experience in a clinical setting.
  • 2+ years of experience engaging and assisting patients approaching end of life - preferably in inpatient (ED, ICU, oncology), palliative, or outpatient geriatric context.
  • Washington State driver's license and vehicle for work-related travel.
Preferred Qualifications:
  • Experience with complex patients and interdisciplinary teams.
  • Some experience in adult education.
  • Knowledge of palliative care best practices.
  • Knowledge of medical terminology and disease courses.
  • Knowledgeable about community resources appropriate to needs of patients/families.
  • Knowledge of culture and values in the patient population and local community.
  • Demonstrated ability to identify and creatively problem solve challenges encountered in care managing patients with complex and serious illness.
  • Proven advanced communication skills: able to establish rapport with patients, families, and colleagues of diverse backgrounds.
  • Proven skilled in building relationships and influencing organizational culture.
  • Proven solid organizational skills to set priorities and ability to work with high degree of accuracy and attention to details while responding to tight deadlines and multiple priorities and demands.
  • Proven skilled in working at the top of scope within discipline, able to integrate cross-disciplinary knowledge when appropriate.
  • Demonstrated ability to productively practice palliative care in complex systems, within ICU or palliative context (or comparable).
  • Proven high EQ person who is flexible and embraces change.
  • Demonstrated ability and willingness to provide emotional support, encouragement, and motivation to individuals and families of varied ethnic, cultural backgrounds, ages, and economic circumstances.
  • Proven fluency in second language utilized in the community.
