Job Summary

We are looking for a highly motivated and experienced RN to join our team as a Nurse Care Coordinator. As a Nurse Care Coordinator, you will work closely with our members to assess, develop, and implement care plans that optimize healthcare outcomes across the care continuum. This is a challenging and rewarding role that requires strong critical thinking skills, excellent communication, and a passion for patient-centered care.

Key Responsibilities
  • Conduct thorough assessments to identify individual needs and develop personalized care plans
  • Collaborate with medical directors and physician advisors to develop care management treatment plans
  • Facilitate authorizations and referrals as needed to ensure timely access to care
  • Monitor and evaluate the effectiveness of care plans and modify as necessary
  • Interface with internal and external resources to meet identified needs
Requirements

To be successful in this role, you will need:

  • A Bachelor's degree in a health-related field and a minimum of 5 years of clinical experience
  • A current, unrestricted RN license in the applicable state
  • Multi-state licensure if providing services in multiple states
  • Excellent critical thinking and problem-solving skills
  • Strong communication and interpersonal skills
  • Ability to work in a fast-paced environment with multiple priorities
Preferred Qualifications

While not required, we prefer candidates with:

  • Certification as a Case Manager
  • Minimum 2 years' experience in an acute care setting
  • Managed care experience
  • Ability to talk and type at the same time
Compensation and Benefits

We offer a competitive salary range of $75,684 to $118,932, based on location and experience. In addition to your salary, you will receive a comprehensive benefits package, including merit increases, paid holidays, Paid Time Off, and incentive bonus programs.
