Estimated Salary Range: $85,000 - $110,000 per year, based on location and experience.
Job Description
The Nurse Care Coordinator will work closely with multidisciplinary teams to assess patient needs, develop care plans, and coordinate home visits. Key responsibilities include:
- Conducting comprehensive patient assessments and developing individualized care plans
- Coordinating with patients, families, and healthcare providers to ensure seamless care transitions
- Maintaining accurate and up-to-date patient records
- Participating in case conference meetings and contributing to care team discussions
Required Skills and Qualifications
To be successful in this role, you should possess:
- A graduate degree from an accredited school of nursing
- A current RN license in good standing
- A minimum of two years of experience as a professional nurse, preferably in a case management or care coordination role
- Excellent communication and interpersonal skills
