Family Nurse Practitioner

PHMC is dedicated to being at the forefront of public health initiatives. It is a requirement for all personnel to be fully vaccinated against COVID-19 by their first day of employment. Vaccination will be provided at no cost through our Health Centers.

Position Overview:


The Family Nurse Practitioner will deliver primary and urgent care services to families within the FQHC Network, focusing on diverse and underserved communities.

Services must align with the health center's mission and quality benchmarks, aiming to enhance patient health outcomes, as well as provider and patient satisfaction, while also strengthening the operational and financial viability of the health center.

The nurse practitioner will report directly to the Medical Director.


Key Responsibilities:

Responsibilities:


  • Deliver primary care services.
  • Operate within all applicable state and federal regulations and the agreement with the collaborating physician.
  • Manage both acute and chronic health conditions utilizing evidence-based protocols.
  • Conduct health screenings, immunizations, and other preventive health programs.
  • Promote the role of advanced practice nurses in underserved regions through advocacy and professional engagement.
  • Inform immediate supervisor of significant issues.
  • Provide age-appropriate assessments and interventions to meet the needs of patients and their families.
  • Offer patient and family education, health counseling, and advocacy services.
  • Be available to work occasional weekends.
  • Administer age-appropriate immunizations.
  • Collaborate with community health nurses for care and follow-up.
  • Participate in public health management of communicable diseases in shelters and other health initiatives as required.
  • Maintain positive relationships with funding entities and community organizations.
  • Perform additional related tasks and responsibilities as assigned.
  • Work collaboratively as part of the Primary Care Medical Home team to provide integrated health services.
  • Document all services promptly.
  • Supervise staff as necessary.
  • Act as a mandated reporter to both the State of Pennsylvania and the Joint Commission.

Qualifications and Skills:

  • Proficiency in engaging resistant clients.
  • Comprehensive understanding of the current healthcare needs of homeless and mentally ill adults.
  • Ability to gather adequate client data, perform physical examinations, interpret findings, and manage or refer patients as necessary.
  • Strong verbal communication and writing skills.
  • Proficient computer skills.
  • Capacity to communicate and work effectively with homeless and mentally ill adults.
  • Ability to prioritize and organize daily tasks to meet the evolving needs of the program and clients.
  • Capability to work within a team framework and respond constructively to team members' concerns.
  • Ability to track work to completion and compile reports.

Education:

  • Must be licensed as an RN and APN Nurse Practitioner.
