HealthMark Rehabilitation operates as an in-house therapy provider, dedicated to serving patients and residents within Skilled Nursing Facilities.

We are currently looking for a PRN Certified Speech Language Pathologist to deliver therapy services to residents and short-term patients.


Key Responsibilities:
  • Deliver therapy services in alignment with physician directives and recognized professional standards.
  • Develop comprehensive therapy care plans that encompass treatment and medical diagnoses requiring skilled intervention, along with planned duration and frequency of services.
  • Establish measurable short and long-term goals that support the discharge plan, adhering to professional practice standards and established guidelines.
  • Execute skilled treatments as per the outlined care plan.
  • Document observations, patient responses to interventions, progress levels, and any significant changes in the patient's condition within designated timeframes.
  • Ensure timely completion of all required documentation in compliance with state and federal regulations.
  • Accurately process billing on the same day services are rendered, adhering to all regulatory policies and guidelines.
  • Assist in maintaining a safe environment in the rehabilitation gym and treatment areas, employing effective infection control methods.

Qualifications:
  • Possession of a valid Texas license/certification for the relevant discipline: CCC-SLP.
  • Strong oral and written communication abilities, along with organizational and analytical skills.
  • Capability to function effectively within multifaceted systems and engage positively with all levels of staff and clients.
  • Demonstrated time management and organizational skills.
  • Proven ability to exercise independent clinical judgment, problem-solving, and decision-making skills.
  • Meet the physical demands necessary to fulfill job responsibilities.
  • Preferred experience in a geriatric rehabilitation environment.
  • Successful completion of the hiring process and background verification.
