We are seeking a skilled and compassionate Admissions Registered Nurse to join our Home Health team at ChristianaCare. As a key member of our team, you will play a vital role in providing high-quality patient care and ensuring a seamless transition from hospital to home.
About the Role:
- Perform and document clinical assessments of patients at time of admission, focusing on their unique needs and circumstances.
- Precept and mentor other nurses to ensure a smooth handover of patient care responsibilities.
- Collaborate with interdisciplinary teams to develop and implement comprehensive care plans that meet the needs of our patients.
- Stay up-to-date with the latest clinical practices and research, incorporating evidence-based care into your daily work.
Requirements:
- Bachelor of Science in Nursing (BSN) degree required; Associate Degree in Nursing (ADN) candidates must acquire their BSN within three years of their start date.
- At least 2.5 years of recent skilled adult home health experience, with a focus on OASIS documentation and proficiency.
- OASIS certification required within 18 months of start date; IV proficiency or willingness to train and achieve proficiency within three months.
