**Job Summary**

James River Home Health & Hospice is seeking a highly skilled and compassionate Hospice Nurse Case Manager to join our team. As a Hospice Nurse Case Manager, you will be responsible for providing high-quality patient care and coordinating with the interdisciplinary team to ensure an optimal care experience for our patients.

**Key Responsibilities:**

  • Conduct initial assessments of patients and families to determine hospice needs
  • Develop and implement individualized care plans that establish goals and incorporate palliative nursing actions
  • Provide coordination and supervision of LPNs and Hospice Aides who support patients
  • Modify and re-evaluate care plans as necessary
  • Monitor, record, and report symptoms and changes in patient condition
  • Obtain and receive physician orders as required
  • Participate in on-call duties as defined by the on-call policy

**Requirements:**

  • Current license to practice nursing in the Commonwealth of Virginia
  • At least one year of hospice experience
  • Graduate of an accredited nursing school
  • Valid driver's license and reliable car available for use during work hours
