**Job Summary**

James River Home Health & Hospice is seeking a dedicated and compassionate Hospice Nurse Case Manager to join our team. As a key member of our interdisciplinary team, you will be responsible for providing high-quality patient care and coordination of services for patients in their place of residence.

**Responsibilities:**

  • Conduct initial assessments of patients and families to determine hospice needs
  • Develop and implement individualized care plans that establish goals and incorporate palliative nursing actions
  • Provide coordination and supervision of LPNs and Hospice Aides as needed
  • Monitor and record patient symptoms and changes in condition
  • Obtain and receive physician orders as required
  • Participate in on-call duties as defined by the on-call policy

**Requirements:**

  • Current licensure as a Registered Nurse in the Commonwealth of Virginia
  • At least one year of hospice experience
  • Graduate of an accredited nursing school
  • Valid driver's license and reliable vehicle available for use during work hours
