Job Summary

Pyramid Healthcare is seeking a highly skilled and dedicated individual to fill the role of Director of Client Support and Care Coordination. This position is responsible for working closely with treatment facilities to ensure seamless coordination of care between levels of care.

Key Responsibilities
  • Collaboration and Communication: Collaborate with facility multi-disciplinary teams to identify gaps in the client care continuum and develop and facilitate plans to improve client care and services.
  • Data Analysis and Reporting: Create and maintain reports of data while working closely with administration to identify problems and implement solutions.
  • Client Navigation: Provide updates and tracking on client navigation through the care continuum to Admissions and Facility management.
  • Stakeholder Coordination: Coordinate meetings among stakeholders to ensure clients move effectively through the continuum of care.
  • Other Duties: Perform other duties as assigned by management.
Required Qualifications
  • Education: Minimum of a Bachelor's Degree or higher.
  • Experience: 5 years of experience in a drug and alcohol and/or mental health setting, preferred.
