Job Summary

As a key member of our clinical team, you will be responsible for providing short-term therapeutic counseling and crisis intervention to patients and families with psycho-social needs.

Key Responsibilities
  • Conduct psychosocial assessments and develop, update, and maintain the Care/Treatment Plan in accordance with facility policy, procedures, Joint Commission standards, and state/federal regulations.
  • Collaborate with the interdisciplinary team to develop and coordinate individualized discharge plans for patients, ensuring a smooth transition to aftercare.
  • Provide group and family process-oriented therapies, as well as educational and didactic groups for patients and family members.
  • Communicate effectively with physicians and other healthcare professionals to report patient progress, needs, and concerns.
  • Develop and maintain thorough knowledge of referral resources throughout the service area.
  • Act as a resource person for other members of the healthcare team, providing in-service education and consultation on social and emotional factors related to health problems.
Requirements
  • Master's Degree in Social Work
  • 3-5 years of experience as a social worker, with mental health experience a plus
  • Licensed Independent Social Worker
