Nurse Field Supervisor

VNS Health is seeking a skilled Registered Nurse (RN) to join our team as a Field Supervisor in Home Health. As a key member of our clinical team, you will play a vital role in delivering exceptional patient care and support.
Key Responsibilities:- Provide case management support, including clinical intervention, development of care plans, and follow-up support to patients in their homes.
- Conduct initial and ongoing supervisory field visits to patients' homes to assess their needs, provide training support, and coordinate care.
- Communicate effectively with the clinical care team to convey any significant changes in patient condition, emergency interventions, or care plan changes.
- Orient and supervise Home Health Aides (HHAs) in the field to ensure high-quality patient care.
- Document patient visits and care plans in the Electronic Medical Record (EMR) in a timely and accurate manner.
- Minimum of two years of experience as an RN in a home care setting.
- Strong clinical and leadership skills, with the ability to work independently and as part of a team.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
