Home Health Physical Therapist Specialist

BAYADA Home Health Care is a leading provider of home health care services, seeking an experienced Physical Therapist to join our team in Maple Shade, New Jersey.
About the Role:We are looking for a skilled and compassionate PT to provide high-quality rehabilitative care to clients in their homes. As a member of our multidisciplinary health care team, you will work closely with nurses, therapists, and other healthcare professionals to develop and implement individualized care plans that promote client independence and well-being.
Responsibilities:- Conduct home visits to assess clients' needs and develop personalized care plans
- Perform diagnostic tests and measurements, such as mobility assessments and strength evaluations
- Implement evidence-based interventions to prevent wound formation and promote wound repair
- Develop and implement manual therapeutic exercises, gait training, and other interventions to improve clients' functional abilities
- Educate clients and their families on rehabilitative care and activities necessary to promote healthy living
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided
- A current NJ Physical Therapist license
- A minimum of one year of recent work experience as a Physical Therapist
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem-solving skills
- Solid computer skills; prior experience with electronic medical records (EMR) preferred
