Key Responsibilities:
- Work collaboratively with our healthcare team to identify patients/residents' strengths, social, emotional, and mental health needs.
- Develop and implement personalized care plans to meet the unique needs of each patient/resident.
- Connect patients/residents and their family members/significant others with community resources and services to support their well-being.
- Provide emotional support and counseling to patients/residents and their families as needed.
Requirements:
- Bachelor's degree in Social Work or a related field.
- Minimum 2 years of experience in social services or a related field.
- Excellent communication and interpersonal skills.
- Ability to work in a fast-paced environment and prioritize multiple tasks.
