Aveanna Healthcare Private Duty Nurse

Aveanna Healthcare is seeking a skilled and compassionate Licensed Practical Nurse (LPN/LVN) to join our team of dedicated healthcare professionals. As a Private Duty Nurse, you will play a vital role in providing high-quality, patient-centered care to individuals in the comfort of their own homes.
Key Responsibilities- Deliver and Coordinate Quality Patient Care: Provide skilled nursing care to patients in accordance with physician orders and developed care plans.
- Assess and Document Patient Needs: Continuously observe and assess patient condition and care needs, documenting all activities, assessments, and nursing actions in a timely manner.
- Implement and Update Care Plans: Participate in the development, implementation, and update of individualized care plans to achieve expected outcomes.
- Provide Education and Support: Educate patients and family members on disease processes, self-care techniques, and prevention strategies to promote optimal health and well-being.
- Collaborate with Healthcare Team: Work collaboratively with the healthcare team, including physicians, case managers, and other healthcare professionals, to ensure seamless care coordination and communication.
- Current LPN/LVN License: Must have and maintain an active, unencumbered license as a Licensed Nurse in the state of practice.
- CPR Certification: Current CPR certification from the American Red Cross or American Heart Association.
- TB Skin Test: Must have a current TB skin test within the last 12 months.
- Experience and Education: Six months of recent experience as a Licensed Nurse in a clinical care setting, with a focus on home health or pediatrics. A degree from an accredited school of nursing is required.
