About the Role:
The Palliative Care Social Worker is a key member of our interdisciplinary team, responsible for providing psychosocial support to patients and their families in various settings, including in-patient, out-patient, and home-based care.
Key Responsibilities:
- Conduct psychosocial assessments and develop individualized care plans to address the physical, emotional, and spiritual needs of patients and their families.
- Collaborate with healthcare teams to provide comprehensive care and support, including facilitating communication and decision-making processes.
- Provide direct patient care, including counseling, education, and advocacy, to ensure the delivery of high-quality palliative care.
- Contribute to staff education, data collection, and quality improvement activities to enhance the delivery of palliative services.
Requirements:
- Master's Degree in Social Work (MSW)
- 3 years of experience in in-patient, out-patient, or home-based social work
- Demonstrated experience in palliative care settings, including hospice and ability to work independently and collaboratively as part of a small interdisciplinary team.
Preferred Qualifications:
- Social Worker License
- Working knowledge of research techniques, data collection, and policy, program, and curriculum development
- Experience with quality improvement methodology
