About the Role

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team as an MDS Coordinator. As a key member of our clinical department, you will be responsible for ensuring the timely and accurate completion of the RAI process and care management process from admission to discharge.

Key Responsibilities
  • Coordinate the RAI process and care management process from admission to discharge in accordance with company policy and procedures, and Federal, State and Certification guidelines.
  • Ensure accurate and timely completion of Minimum Data Set, discharge, and admission tracking.
  • Collaborate with the Director of Nursing and VP of Clinical Reimbursement to coordinate information systems operations and education for the clinical department.
  • Develop and maintain a thorough understanding of the Quality Indicator process, OBRA regulations, and Minimum Data Set.
  • Provide professional guidance and support to the care planning process.
Requirements
  • Current licensure as a Licensed Practical Nurse (LPN) in the state of Missouri.
  • Prior MDS experience (highly preferred).
  • Minimum of one year experience in an LTC/SNF/AL/MC setting (highly preferred).
  • Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement (highly preferred).
  • Professional image in both appearance and behavior.
