Nurse Practitioner

CodeForce is seeking a skilled Licensed Practical Nurse (LPN) to join our primary care team in an ambulatory care setting. As a key member of our team, you will provide patient care according to established standards of care, collaborating with Care Managers and Medical Providers to ensure safe, timely, effective, efficient, equitable, and patient-centered care.
Responsibilities- Provide patient care in the ambulatory care setting according to established standards in collaboration with Medical Providers, Registered Nurses, and Medical Assistants as members of the care team.
- Provide primary support to the provider by proactively anticipating the provider's needs, preparing the patient for the visit, maintaining efficient patient flow, and assisting with treatments and procedures.
- Ensure safe, timely, effective, efficient, equitable, and patient-centered care of patients.
- Minimum 1-year experienced nurse from an accredited practical nursing program.
- Competencies in patient safety, team care, caring practices, evidence-based practices, patient education, response to diversity, systems resources, and clinical reasoning.
- Essential skills: BLS.
- Non-essential skills: ability to effectively interact with customers, Microsoft Office, written communication, and demonstrated ability to communicate effectively in person and via telephone.
