Care Manager Job Description
We are seeking an experienced Registered Nurse (RN) to join our team as a Care Manager for Home Health patients in the Bay Area. As a Care Manager, you will be responsible for creating and implementing individualized plans of care for patients in their homes. This includes assessing patients' needs, developing rehabilitation plans, and collaborating with interdisciplinary teams to deliver high-quality care.
About the Role
- You will work with a dedicated team of healthcare professionals to provide comprehensive care to patients in their homes.
- Your primary responsibility will be to assess patients' needs, develop rehabilitation plans, and collaborate with healthcare teams to deliver high-quality care.
- You will also be responsible for educating patients and families on wound care, medication management, and other aspects of home health care.
Requirements
- You must be a licensed RN with current and unrestricted California licensure.
- You should have experience working with high-acuity patients and be familiar with home health care protocols.
- You must possess excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
