Housecalls Nurse Practitioner

We are seeking a skilled Housecalls Nurse Practitioner to join our team in Jasper, Newton, and Sabine, TX. As a member of our Home and Community Care team, you will provide annual clinical assessments to patients in the comfort of their homes, helping to identify and reduce health risks and coordinate follow-up care to improve health and well-being.
Key Responsibilities:- Conduct in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams.
- Perform evidence-based practice screenings, including point-of-care testing as appropriate.
- Identify diagnoses to inform care management and active medical management.
- Communicate findings to inform members' primary care physicians of potential gaps in care.
- Educate members on disease processes, medication, and compliance.
- Active NP license or ability to obtain by start date, with unencumbered licensure.
- Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or ability to obtain by start date.
- Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions.
- Reliable transportation for home visit assessments.
- 1+ years of clinical experience in the highest level of education, clinical setting.
- May be requested to obtain additional licensure in other geographic areas.
- Experience in gerontology, cardiology, internal medicine, or endocrinology.
- Home health care or home visit experience.
- Ability to travel throughout the state, beyond your assigned region, up to 5% of the time.
