Gentiva Hospice is seeking a skilled Palliative Care Nurse Practitioner to join our team. As a Palliative Care Nurse Practitioner, you will play a critical role in providing high-quality, patient-centered care to individuals with serious illnesses.
Key Responsibilities- Assess patients' physical, emotional, and spiritual needs, and develop individualized care plans to address these needs.
- Provide diagnosis, treatment, and follow-up care to patients, working collaboratively with the Palliative Medical Director and other members of the palliative care team.
- Facilitate communication between patients, families, and healthcare providers to ensure seamless care coordination.
- Participate in evening and weekend call as required, providing on-call services in a clinically competent and responsive manner.
- Prescribe medications, including controlled substances, as delegated and licensed, and order laboratory and diagnostic tests as needed.
- Master's degree in Nursing from an accredited school of nursing.
- Minimum of two years of experience as a Nurse Practitioner, with a focus on palliative care.
- Current license as a registered nurse and current Nurse Practitioner certification/license.
- Collaborative agreement with a supervising physician, as required by state law.
- Part B Provider certification and Federal DEA license.
