Primary Care Nurse Practitioner

We are seeking an experienced Physician Assistant or Nurse Practitioner to join our practice full-time in Madison, WI. As a key member of our healthcare team, you will be responsible for managing inpatients at a brand-new LTACH in the Madison, WI area. This role requires a compassionate provider who is comfortable managing acute changes in place and has expertise in primary care, acute care, and chronic care practices.
Responsibilities:- Conduct thorough history and physical exams, examining patients and their medical records, pertinent documents, imaging, labs, and tests.
- Order and study diagnostic tests, including blood tests, X-rays, etc.
- Diagnose, manage, and treat acute episodic illnesses and stable chronic conditions, evaluate current therapies, health maintenance, health promotion, illness prevention education, and counseling.
- Prescribe medications and write referrals as needed.
- Maintain accurate records and schedules, establishing and maintaining a system for tracking and coordinating patient caseloads and their progress throughout the acute or chronic episode of care.
- Inform patients about their health conditions and prescribed medications, educating them and their family members on diseases, epidemics, and prevention.
- Collaborate with the healthcare team, monitoring, evaluating, and communicating relevant data regarding clinical problems to physicians and other team members.
- Oversee patients' overall health, identifying opportunities for each patient's needs relative to their age, monitoring progress, and collaborating with other team members to ensure continuity of care.
- Promote good health practices and identify potential deterrents to compliance with treatment regimes.
- Advance professional knowledge and conduct research, following up with patients if they are discharged to assess if needs are being met and determine if additional follow-up care is required.
- Minimum 3 to 5 years of acute inpatient hospital experience as a Physician Assistant or Nurse Practitioner, with the ability to work autonomously.
- Valid Nurse Practitioner or Physician Assistant license in WI.
- Master's degree from an accredited NP or PA program.
- ACLS certification.
- Total compensation range: $115k-$120k per year, plus potential productivity bonus.
- Vacation/PTO: 120 hours of Paid Time Off annually accrued.
