Key Responsibilities- Provide accurate and comprehensive assessments, diagnoses, and treatment plans for patients with a range of medical conditions.
- Develop and implement individualized care plans, taking into account patient preferences, values, and goals.
- Collaborate with the clinical care team to ensure seamless care coordination and communication.
- Participate in clinical decision-making, including ordering diagnostic tests and prescribing medications as needed.
- Stay up-to-date with the latest evidence-based practices and guidelines in primary care.
- Master's degree in Nursing with a focus on Family Nurse Practitioner or a related field.
- Current National Board Certification as a Family Nurse Practitioner.
- Active Advanced Practice Registered Nurse (APRN) license in good standing.
- Minimum of 1 year of recent primary care experience as a Nurse Practitioner, or successful completion of CVS Minute Clinic primary care training program.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Strong problem-solving and critical thinking skills, with the ability to prioritize tasks and manage multiple responsibilities.
