Senior Community Care Nurse Practitioner/Physician Assistant

In this role, you will provide high-quality, patient-centered care to seniors in their homes, nursing homes, and assisted living facilities. You will work collaboratively with our interdisciplinary team to develop and implement personalized care plans that address the unique needs and goals of each patient.
As a Senior Community Care Professional, you will have the opportunity to work with a diverse range of patients, including those with complex medical conditions and those who require end-of-life care. You will be responsible for conducting comprehensive assessments, developing and implementing care plans, and coordinating with other healthcare professionals to ensure seamless care transitions.

Key Responsibilities:
- Conduct comprehensive assessments of patients to identify their unique needs and goals
- Develop and implement personalized care plans that address the physical, emotional, and social needs of patients
- Coordinate with other healthcare professionals to ensure seamless care transitions
- Provide high-quality, patient-centered care to patients in their homes, nursing homes, and assisted living facilities
- Collaborate with our interdisciplinary team to develop and implement care plans that address the unique needs and goals of each patient
Requirements:
- Current licensure as a Nurse Practitioner or Physician Assistant
- Experience working with seniors and individuals with complex medical conditions
- Strong communication and interpersonal skills
- Ability to work collaboratively as part of an interdisciplinary team
- Strong problem-solving and critical thinking skills
Preferred Qualifications:
- Experience working in home and community care settings
- Knowledge of geriatrics and chronic illness management
- Ability to develop and maintain positive relationships with patients, families, and healthcare professionals
