Job Title: Registered Nurse

We are seeking a highly skilled and compassionate Registered Nurse to join our team at Caregiver, Inc. As a Registered Nurse, you will play a vital role in providing exceptional care to individuals with intellectual and developmental disabilities.

Responsibilities:
  • Provide direct and individualized nursing care to a caseload of consumers, including those receiving Wellness Coordination Services.
  • Ensure the prescribed medications and treatments are available in the home for each consumer.
  • Develop, document, implement, and evaluate health care protocols and nursing care for each consumer, as required by the Individual Support Plan (ISP).
  • Act as a resource for agency LPNs as assigned.
  • Monitor vital signs, weight charts, immunization records, and health status of DSA consumers.
  • Review the agency medication error report and make recommendations to the Executive Council.
  • Manage the agency Psychotropic Medication Review Clinic as assigned.
  • Participate in Area Individual Support Team (IST) meetings, sharing knowledge, information, and ideas among team members.
  • Record information concisely, accurately, and completely.
  • Ensure consumer's medical information chart is current.
  • Maintain a positive working relationship with family members and guardians of DSA consumers.
  • Serve as the primary contact when a consumer is hospitalized, communicating information between the agency staff and hospital staff.
  • Assist in the development of staff training curriculums, including training in detecting signs of illness or dysfunction that warrant medical or nursing intervention, basic skills required to meet the health needs of the consumers, Core A & B curriculum, CPR, and first-aid in the presence of an accident or illness.
  • Administer and read TB skin tests as necessary for consumers and staff.
  • Ensure nursing assessments are completed per agency policy.
  • Serve as a liaison between health care allied service providers and the Area Directors.
  • Ensure consumer medical records are maintained in a manner consistent with HIPAA regulations, other Federal and State regulations, and applicable DSA, Inc. policies and procedures.
  • Participate in the nursing on-call schedule.
  • Serve as a DSA, Inc. representative to trade organizations and state or regional committees.
  • Participate in routine supervisory reviews with the Clinical Services Manager.
  • Appoint a person to act on her/his behalf during absences.
  • Other duties as assigned.
Qualifications and Skills:
  • Degree from an accredited school of nursing.
  • Current state licensure as a licensed nurse.
  • Five years of clinical practice preferred.
  • BSN preferred.
  • One year of experience working with individuals with developmental disabilities or in a related field preferred.
  • Maintains credentials necessary to teach Core A/B courses.
  • Maintains certification necessary to administer and read tuberculosis skin tests.
  • Must be at least twenty-one (21) years of age.
  • Ability to communicate effectively; orally and in writing.
  • Ability to use common word-processing, spreadsheet, and communications software programs within a Windows environment.
  • Satisfactory completion of criminal background checks as required by federal and/or state laws and any other post-offer pre-employment screenings as mandated by a governing agency or DSA, Inc.
  • Valid Indiana driver's license, a good driving record, and a current automobile liability insurance policy are required.
