Patient Flow Coordinator Team Lead

We are seeking a highly skilled and experienced Patient Flow Coordinator Team Lead to join our team at Ballad Health Corporate. The successful candidate will be responsible for overseeing the patient bed management queue, documentation, and staff assignments.
Key Responsibilities- Manage patient bed placement workflows and ensure compliance with hospital policies and procedures.
- Coordinate with medical staff and other members of the interdisciplinary team to facilitate patient movement within the system.
- Monitor and control noise levels in the call center and promote teamwork and morale.
- Work closely with Transfer Center, Comms Center, facility leadership, Behavioral Health, and LTC to place patients in a timely manner.
- Collaborate with house supervisors, facility CNOs, transfer coordinators, and RCMCC coordinators to manage patient admissions, transfers, and placement.
- Utilize real-time capacity planning dashboards to make informed decisions about patient placement.
- Communicate ongoing changes and challenges to the Patient Flow Clinical Lead or Manager.
- Prepare end-of-shift reports and maintain accurate records.
- Bachelor's Degree – RN or LPN with 5 years of experience in a clinical environment.
- Excellent communication and leadership skills.
- Ability to work in a fast-paced environment and prioritize tasks effectively.
- Strong problem-solving skills and ability to think critically.
