Assertive Community Treatment Clinician

About the Role
We are seeking a dedicated and compassionate Licensed Social Worker - ACT to provide high-quality care to individuals with severe mental illness. As an integral member of our multi-disciplinary team, you will work closely with clients, their families, and community partners to assist them in achieving their treatment and recovery goals.
The successful candidate will have excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team. A strong understanding of evidence-based treatments, including cognitive behavioral therapy, harm reduction, and trauma-informed care, is also essential.

Key Responsibilities:
- Collaborate with a multi-disciplinary team to deliver integrated services
- Participate in daily team meetings to review and plan client services
- Provide 24-hour crisis intervention on-call services on a rotating basis
- Complete required documentation in a timely and high-quality manner
Requirements:
- Bachelor's or Master's degree in Social Work (MSW)
- 2+ years of experience working in mental health or related field
- Strong communication and teamwork skills
- Ability to work in a fast-paced environment and prioritize tasks effectively
