Job Summary:

Eden Health is seeking a skilled Registered Nurse to provide in-home care services to our patients in Whatcom & Skagit Counties. As a Home Health Registered Nurse, you will be responsible for providing admission, case management, and follow-up skilled nursing visits to our patients.

Key Responsibilities:
  • Provide hands-on care, management, and evaluation of the care plan and teaching of the patient in accordance with physician orders.
  • Revise plan in consultation with physician based on ongoing assessments and as required by policy/regulation.
  • Coordinate appropriate care, encompassing various healthcare personnel, including Physical Therapists, Occupational Therapists, Speech Therapists, Social Workers, Home Health Aides, and external providers.
  • Report and document patient care/condition/progress to patient's physician and Clinical Manager on a continuous basis.
  • Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems.
  • Coordinate/oversee/supervise the work of Home Health Aides and provide written instructions that reflect current plan of care.
  • Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.
Requirements:
  • Graduate of an accredited nursing program.
  • Current licensed Registered Nurse (RN) in the state of practice.
  • Experience as an RN with one of the following preferred: one year of experience in an acute-care setting within the last 24 months, or six months of Home Health experience within the last 12 months.
  • Credentials in specialty areas preferred (wound care, OASIS certification, IV, ET).
  • Valid driver's license and reliable transportation.
  • Proof of automobile insurance (MVR checked).
  • Proof of current CPR certification.
  • Able to pass criminal background check and national sex offender clearance.
