Job Summary

We are seeking a skilled LPN Home and Community Health Professional to join our team at PeaceHealth. As a key member of our home health care team, you will provide high-quality patient care and support to individuals in their homes.

Key Responsibilities
  • Implement and evaluate patient care plans under the supervision of a registered nurse.
  • Provide direct patient care in patient homes, including administering medications and treatments as prescribed.
  • Assess patient needs and develop individualized care plans to meet those needs.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
  • Document patient care in accordance with department standards.
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care.
Requirements
  • Accredited State Board of Nursing LPN Program required.
  • Minimum of 1 year of clinical experience as an LPN preferred.
  • Demonstrated knowledge of and ability to apply age-specific principles of growth and development and life stages to meet each patient's needs.
Preferred Qualifications
  • Basic computer skills, including familiarity with MS Office applications.
  • Cooperative working and interpersonal skills, with the ability to work effectively with a wide variety of patients, families, and staff.
  • Ability to communicate effectively with individuals diagnosed with mental and behavioral disorders, and to operate efficiently in an independent home setting.
Working Conditions
  • Lifting: Patient handling no greater than 35 lbs. without the use of assistive equipment and/or devices.
  • Fine motor skills to grasp and control medical equipment and perform precise procedures.
  • Push/Pull: Frequently up to 45 lbs. force.
  • Ability to move around area with frequent sitting.
  • Bending/ stooping/ squatting/ reaching/ kneeling frequently.
Environmental Conditions
  • Exposure to biohazard, body fluids, and airborne particles.
  • Ability to complete tasks in a noisy environment.
Mental/Visual
  • Vision and hearing required within normal limits.
  • Ability to communicate and exchange accurate information.
