We are seeking a skilled Licensed Vocational Nurse to join our Hospice team at ABL Health Care, LLC.
Key Responsibilities:
- Deliver high-quality patient care in the comfort of their own homes under the supervision of a Registered Nurse.
- Travel to patients' homes to provide skilled nursing services.
- Maintain accurate and timely clinical notes, orders, and charting.
- Collaborate with physicians and agency staff to ensure seamless care delivery.
- Work closely with patients and their families to achieve health goals.
- Manage patient health care episodes with the agency.
- Requires 1 year of experience in Hospice care.
- Reliable transportation and a valid Driver's license are necessary.
- LVN License is required.
- Optional on-call duties after office hours and on weekends.
