Job Summary

Self-Help for the Elderly seeks a skilled Licensed Vocational Nurse to provide high-quality home health and hospice care to patients and their families. As a key member of our interdisciplinary team, you will work closely with Registered Nurse Case Managers to develop and implement individualized care plans that promote comfort, health, and well-being.

Key Responsibilities
  • Assist Registered Nurse Case Managers in ensuring the safe and effective delivery of home health and hospice nursing services.
  • Perform nursing care plans as directed by Registered Nurse Case Managers, including nursing services, treatments, and preventative and rehabilitative procedures.
  • Obtain and incorporate data on physical, psychological, social, and spiritual factors that may influence patient health status into the plan of care.
  • Coordinate physical care of patients by teaching and/or training patients, primary caregivers, volunteers, and employed caregivers.
  • Communicate regularly with Registered Nurse Case Managers regarding changes in patient condition and/or needs.
  • Complete patient-related documentation within 24 hours of the shift worked and maintain up-to-date patient records.
  • Collaborate and consult with the interdisciplinary team on issues related to patient care.
  • Participate in in-service training programs for professional staff and quality assurance and performance improvement activities.
Requirements
  • Current California Licensed Vocational Nurse.
  • Minimum one year professional nursing experience in home health, hospice, acute, or sub-acute settings preferred.
  • Able to work both independently and as part of an interdisciplinary team.
  • Good communication and interpersonal skills.
  • Bilingual in English and Cantonese preferred; Mandarin and Toishanese a plus.
  • Must pass health screening for TB clearance, physical examination, and background check.
  • Current CPR certification.
  • Must be able to travel via public transportation or personal vehicle for performance of job duties.
