Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our interdisciplinary team. As a Hospice Nurse, you will play a vital role in providing high-quality care and support to our patients, working closely with our team of healthcare professionals to ensure the best possible outcomes.

Key Responsibilities
  • Provide comprehensive nursing care to patients in their homes, skilled nursing facilities, and assisted living facilities.
  • Conduct thorough psych-social assessments, develop and implement individualized care plans, and provide ongoing support and education to patients and their families.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Manage patient caseloads, prioritize tasks, and maintain accurate records and documentation.
  • Participate in ongoing education and training to stay up-to-date on the latest hospice care practices and regulations.
Requirements
  • Current Registered Nurse licensure (in good standing) in the state(s) of practice.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Strong time management and organizational skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Ability to learn and utilize electronic medical records systems.
  • Valid driver's license and reliable transportation, with a willingness to travel across the service area.
