Triage Nurse Specialist

The Telephone Triage Nurse plays a critical role in managing and responding to time-sensitive, symptom-based calls received by Zufall Health Center. This position requires a skilled nurse to handle telephone nursing practice, focusing on assessment, prioritization, and referral to the appropriate level of care.
Key Responsibilities- Consistently adhere to departmental policies and procedures.
- Conduct all aspects of the job in a professional and ethical manner.
- Work independently with minimal supervision, utilizing available resources for problem resolution as needed.
- Maintain high levels of confidentiality and HIPAA compliance.
- Collaborate with team members to ensure optimal outcomes of care and service.
- Utilize departmental resources prudently and appropriately.
- Communicate effectively both verbally and in writing, using appropriate mechanisms for identifying and resolving work-related issues.
- Keep manager or supervisor informed of work-related issues at all times.
- Perform additional duties as determined by supervisor.
The Telephone Triage Nurse will interact with patients and their representatives, gathering information and determining the nature and urgency of their healthcare needs. This will involve identifying and verifying problems, planning, decision-making, and output disposition, including scheduling appointments or directing patients for immediate care.
The nurse will maintain a process for telephone nursing, including information gathering, processing, and output disposition. They will also receive and document all explicit and implicit information, distinguishing high and low acuity calls, complexity, and contextual factors.
Requirements- Bachelor's Degree in Nursing required.
- Minimum of two years clinical experience in an ambulatory care setting or equivalent experience.
- Bilingual English and Spanish required.
- Knowledge of computer software programs such as Microsoft Word and electronic medical records.
- Critical thinking, competency, and standard judgment.
- Strong communication and customer service skills.
- Ability to work with minimal supervision and adapt quickly to changes in workflow or process.
